Healthcare Cost Savings Estimator Tool (the “Tool”) and ebp-savings.info are tools designed to estimate cost savings by averting ER visits and hospitalization among evidence-based program participants. This web-based estimator has built-in features allowing users to adjust calculations based on site-specific data.

Given that this local data is available to you, please be sure to click the "CDSMP Delivery Cost per Person" checkbox on the "Before You Begin" checklist. This will allow you to enter the average per participant cost associated with delivering CDSMP in your local CDSMP evaluation.
In this step, you are asked to provide the number of participants ages 18 and older that you expect to enroll in CDSMP workshops in the next 12 months.
Then, based on this number, you are asked to estimate the age distribution for participants based on the following age groups: 18-44 years, 45-64 years, and 65+ years. The percentages for the three age categories should sum to 100%.
